Certified Nursing Assistant CNA - Acute Care Skilled Nursing $23.71 per hour

Clinician must be aware that this facility and shift is Subacute Specialty focused and clinician is agreeing to the following:

1. Subacute experience is highly recommended
2. Check in at Nursing station upon arrival.
   1. Check in at facility nursing station is required before checking in on the Nursa app
   2. IF FIRST TIME AT FACILITY GET ORIENTATION PACKET FROM NURSE SUPERVISOR.  Required for first day at the facility 
3. Please arrive to your shift on time. If you are going to be late, please call the facility and give notice to the DON.
   1. Additionally, if you arrive late your Nursa shift report must be reflective of the time you arrived.
   2. The same goes for early departure. If you leave your shift early, it must be approved by the charge nurse and the Nursa shift report must be reflective of your early departure.
   3. Conflicting times in shift report details as it relates to arrival and departure can result in denial of future shift requests.
4. Complete all PCC charting and documentation prior to end of shift. Pay is subject to be reduced or with-held otherwise.
   1. Try logging in with your PCC credentials at the beginning of your shift.  If you're unable to login, get this corrected right at the beginning of your shift.
5. Scrubs are the desired attire.
6. Complete the 530 form before you leave.
   1. The 530 form binder is located near the Nurse's station
   2. Properly list your correct assignment and patient rooms assigned to you ON THE FORM.
7. Please take your required 30-minute break before the 5th hour.

ANY CLINICIAN REQUESTING SHIFTS AT BUENA PARK NURSING CENTER IS ACKNOWLEDGING AND AGREEING TO THE FOLLOWING:

I, CLINICIAN NAME, am recognizing and agreeing to complete ALL duties of a CNA. This includes responsibilities related to patient care INCLUDING documenting care, or otherwise known as “charting”, in PCC. 

As a 1099 clinician it is still MY responsibility to complete charting that is done for patients while under my supervision. Facilities are required to provide MY documentation when audited by state and federal compliance organizaitons. IT IS MY DUTY TO CHART and I will not end my shift or leave the facility until ALL CHARTING IS COMPLETE.

Failure to comply with proper documentation or “charting” can result in the following:

1. Delayed Payment
2. Requirement to come back to facility to complete charting
3. Nursa Suspension (inability to complete future shifts at any facility in the Nursa Application)
4. Blocked from Facility
