Nurse and Healthcare Workers Compliance Documents and Requirements
Registered nurses (RNs), licensed practical nurses (LPNs), and certified nursing assistants (CNAs) connect with local PRN jobs on Nursa, a modern approach to nurse staffing and recruitment.
Requirements by facilities vary not only by license and position but by nature of the setting as well. RN job requirements will vary from CNA job requirements or LPN job requirements, although there will be commonalities as well. Regardless, we've put together an all-inclusive list. Standard Contract/PRN Nurse Docs
- Driver’s License and/or Passport - A valid driver’s license will be required, you'll need to color scan both the front and back and save as a PDF to upload. If you don't have a valid driver's license, then color scans of your passport can be used.
- SSN Card - Your social security card needs to be signed. You will need to scan the front and back of the card. If you cannot locate your social security card, you can request one and provide us with a letter from the social security office that your card is being processed.
- Education Diplomas - This can include your high school graduation diploma or GED equivalent, your associate's degree or bachelor’s degree from a school of nursing. NursaTM uses PeopleTrail and the National Student Clearinghouse to verify your education. You will be expected to provide the school name, address, and dates attended.
- Background Checks - NursaTM will cover the costs of background checks. Some facilities require more thorough checks than others, and yet other facilities do background checks through a manual name search as opposed to an electronic search. Except in cases of delays, these will typically take only 24-48 hours. Background checks will include the GAS/SAM, OIG, and the National and State Sex Offender Registries.
- References - It is standard for facilities to want three references. References should be from supervisors or managers and at least one from your most recent employment.
- Fit to Work Statement or Physical - A document stating clearly that you are "fit for work" is required. If you don't have a fit for work statement, a physical is also appropriate. If a new physical is requested, NursaTM will cover the cost.
- Work History - Up to seven years of work history is required. Be sure to include the name and address of the facility in which you worked along with the name of the agency for each assignment.
- Resume - Your resume should include your work history going back seven years. Explanations for gaps in your work history are expected.
Competency Exams and Skills Checklists for PRN Nurses & CNAs
Competency exams are not required by all facilities, but when they are, will be sent through the platform Prophecy. You will receive login instructions to complete the requested assessments and your results can be converted to a PDF for your records. Skills checklists may also be requested by certain facilities in order to assess your abilities applicable to the posting.
License Verification for CNAs, RNs, and LPNs
All nurses and assistants must verify their license in the state in which they work:
- CNA certifications will vary by state and will be required for each state you wish to work in.
- RN and LPN licenses need to be for whichever state you wish to work in, however a compact nursing license is appropriate as well, and should be considered as by travel nurses to ease transitions from one location to another nationwide.
Basic Required Certifications for Nurses & Healthcare Professionals
Certifications in this category must be through the American Heart Association (AHA). Be sure your certification is current, if you don't have these certifications it will be important to get enrolled in a class right away. If you aren't sure the status of your certification, be sure to check because you may need to renew.
- BLS - Basic Life Support
- ACLS - Advanced Cardiovascular Life Support
- PALS - Pediatric Advanced Life Support
Crisis Prevention Intervention Training for PRN Nurses
CPI certification may be required for some facilities for certain postings. This training equips you with knowledge and tactics to deescalate disruptive behaviors. If you're applying for PRN nursing jobs of a managerial or supervisory position, this training is recommended, even if not required.
Vaccinations and Titers for Required for Most Healthcare Professionals
Vaccines and titers are vitally important for healthcare workers. This list of vaccinations and titers should be read carefully. If you decline any of these vaccinations due to a health or religious reason, you will need declination forms for each.
- Hepatitis B - Documentation of the three-series vaccination and titer paperwork required.
- MMR - Documentation of the two series vaccinations and titer paperwork required.
- Varicella - Documentation of the two series vaccinations and titer paperwork required.
- Influenza - Documentation of this vaccine is typically required by facilities.
Mask Fit Test for Healthcare Workers
The test verifies your ability to have a proper fit for masks or respirators such as:

- 3M N95 1870
- Kimberly Clark Duckbill
Neonatal Resuscitation Program for Nurses (NRP)
NRP certification from the American Academy of Pediatrics may not be required for all postings, but for any position where you may have contact with infants this is highly recommended.
Trauma Nursing Core Course (TNCC)
The Trauma Nursing Core Course is fit to prepare you to respond to urgent circumstances to utilize your knowledge, critical thinking skills and apply them practically under pressure.
Tuberculosis Screening & Documents: PPD, Tdap, or Chest X-rays and Titers
- The PPD are the skin tests (TB1 and TB2) for tuberculosis and need to be periodically performed throughout a healthcare worker's career. The cost for these is covered by NursaTM
- The IGRA, Tspot, and QuantiFERON are various forms of the tuberculosis titer. Different facilities will require different titers. For these services NursaTM will cover the costs.
- The Tdap booster immunization needs to be done on a ten-year cycle. Unlike other tests and titers tuberculosis relation, this cost is not covered by NursaTM.
- A Chest X-ray may be required by some facilities.
