Please arrive at the location on time to receive report from the off-going shift. Clock in as soon as you arrive in the facility. Your 9digits Registry ID number is 3******** (Ex. 300000000) Please keep this assigned registry number with you at all times you picked up a shift at Autumn Hills. Click "Nursa" for the registry. Make sure you complete your punches to avoid problems with invoice. If you are not registered in our clock because this is your first shift with us, please see the DSD or Staffing Coordinator immediately to get your Agency number and be added to the time clock biometrics for future shifts. If you report for a weekend or NOC shift for the first time and the DSD or Staffing Coordinator are not available, you MUST fill out a time correction form. This will prevent any issues with payment for your shift. For any questions you can ask the receptionist. Please make sure to sign the 530 Form and Nursing Sign-in Sheet at Station 1. Sign in the slot which matches the assigned run and room numbers. Don't forget to put the meal breaks. For POC charting, the last 4 digits of your social security will be your password/pin. If you are not in the system, we have a generic username and password available for you or the RN Supervisor can call IT Fundamentals to get access for you. Please don't hesitate to ask nurses if any question. Address: 430 N. Glendale Avenue, Glendale CA 91206 Tel: (818) 246 - 5677. PARKING: Please park on the street and check the posted signs. Our parking lot is for family parking during visiting hours. Thank you for your cooperation. We are located next to Chase bank at Glendale and Doran. Thank you :)
