Shift Instructions
FACILITY CLINICIAN EXPECATIONS
- A NEW NURSA CHECK-IN PROCESS IS REQUIRED AT THIS FACILITY (MUST adhere to be ONE-TIME PASSCODE check-in process to receive pay AND be invited back)
- ONLY clinicians with at least 1 year or more of SNF experience will be considered (experience must be in Nursa profile or resume added on Nursa profile OR resume submitted to [email protected])
- ONLY clinicians who have completed the youtube video on Matrix overview will be considered (video link certification is a facility requirement in Shiftredy- please complete verification)
- ONLY clinicians who enable locations services in the Nursa app will be considered
- ONLY clinicians who have completed Nursa inservice (facility requirement and certification available on Shiftready- please complete verification) will be considered
- ONLY clinicians with valid clinical license, valid Government ID, and complete Nursa profile will be considered for shifts at this facility
Arrival Instructions
- Park Silverado Memory Care parking lot (shared parking) at enter facility through the main entrance
- Sign in at kiosk
- Check in at the Nursing station upon arrival (a NEW NURSA CHECK-IN/CHECK-OUT PROCESS is required at this facility. YOU MUST follow the instructions in bullets 2 and 3)
- The Nursing station is where the Nursa Display will prompt a new check-in/check-out process through a “One-Time Passcode”
- Follow the prompts in the Nursa app to complete check-in and official arrival
- Obtain a physical copy of your 530 form from Gerlie Vergara (staffer) or Head Nurse and provide a wet signature
- Retrieve your matrix access information and assignment from Gerlie (staffer) or Head Nurse
- IF THIS IS FIRST TIME AT FACILITY, retrieve orientation from RN supervisor ONLY ON FIRST DAY AT FACILITY
- Please arrive to your shift on time. If you are going to be late, please call the facility and give notice to the staffer or head nurse.
- If you arrive late your Nursa check-in process will automatically record your late arrival time when using the “One-Time Passcode” process
- When you leave your shift you must use the Nursa check-out process at the Nursing Station using the Nursa Display and follow the prompts in the Nursa app for check-out process using the “One-Time Passcode”
- Clinicians who do not use the Nursa “One-Time Passcode” process will not be invited back to The Dorothy and Joseph Goldberg Healthcare Center and pay will be delayed or withheld
- Complete all Matrix charting and documentation prior to end of shift. Pay is subject to be reduced or with-held otherwise.
- Try your Matrix access at the beginning of your shift to get it correctly logged in with no issues
- Scrubs are the desired attire.
- Complete the 530 form before you leave.
- The 530 form binder is located near the Nurse's station
- Properly list your correct assignment and patient names and rooms.
- Please take your required 30-minute break before the 5th hour.
ANY CLINICIAN REQUESTING SHIFTS at THE DOROTHY AND JOSEPH GOLDBERG HEALTHCARE CENTER IS ACKNOWLEDGING AND AGREEING TO THE FOLLOWING:
I, CLINICIAN NAME, am recognizing and agreeing to complete ALL duties of a CNA. These responsibilities are specific to the state of CA regarding patient care INCLUDING documenting care, or otherwise known as “charting”, in Matrix.
As a 1099 clinician it is still MY responsibility to complete charting that is done for patients while under my supervision. Facilities are required to provide MY documentation when audited by State and Federal compliance organizations. IT IS MY DUTY TO CHART and I will not end my shift or leave the facility until ALL CHARTING IS COMPLETE.
Failure to comply with proper documentation or “charting” can result in the following:
- Delayed Payment
- Requirement to come back to facility to complete charting
- Nursa Suspension (inability to complete future shifts at any facility in the Nursa Application)
- Blocked from Facility
