1. Upload Your Government Identification Card
Any valid (aka not expired) government identification card is accepted for this step. This ID can be your driver’s license, a DMV-issued non-driver card (state ID), or your passport.

How?

2. Upload Your CPR/First Aid Certification
For registered nurses and licensed practical nurses, this step is the appropriate placement for your Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications. If you have it, a Pediatric Advanced Life Support (PALS) certification is also appropriate here.
For certified nursing assistants, your Cardiopulmonary Resuscitation (CPR) certification from the American Heart Association (AHA), the American Red Cross, or your local fire department should be uploaded here.

Vaccine exemptions are typically categorized as either religious or medical. Specific forms or examples for a religious or medical vaccination exemption letter may be found on your state’s health department website or by contacting your local health department clinic.
How?
When taking a photo of your vaccination card or a COVID-19 vaccination exemption form, please ensure the entire document is in the frame. If you are uploading an exemption, please upload under other credentials "COVID-19 Vaccine Exemption" and not essential credentials.
