Bradford Oaks asks clinicians to sign an independent contractor agreement before their first shift at the building. Autumn Lake Healthcare treats it as a credential, so it sits on your Nursa profile next to your license and your other nursing credentials rather than in a folder at the facility.
What does signing commit you to?
Less than clinicians tend to assume. The agreement establishes how you work when you work there, not that you will. No minimum number of shifts comes attached to it, no schedule you are agreeing to fill, and no penalty for going weeks without picking anything up at Bradford Oaks. Signing puts you in a position to accept shifts, and accepting stays a separate decision each time.
Where do pay and shift details come from?
Not from this document. The agreement covers the working relationship in general terms, while the specifics of any one shift, including its rate, its hours, and its unit, come from the posting you accept. A rate that does not match what you expected is a question about that shift rather than about the agreement.
Should you keep your own copy?
Yes. Uploading it to Nursa handles the credentialing side, but a signed agreement is also part of your own records as a 1099 contractor. Keep it with:
- The 1099 forms you receive each year
- Your license and certification documents
- Any facility agreements you have signed elsewhere
What happens after you sign?
Verification comes next, and the document does not count until it clears. Bradford Oaks shifts stay out of view until then, which catches clinicians who sign and start searching the same hour. Check the credential status on your profile rather than treating the upload as the finish line.

No test, no certificate. The signed agreement is the credential.
- Tap Menu in the top left of the Nursa app.
- Select Profile.
- Scroll to the Credentials section.
- Tap Add your Credential.
- Upload your signed agreement as a PDF, WORD, JPG, or PNG.
