Emergency Contact Form

An emergency contact form records who a facility should call if something happens while you are working. It takes 2 minutes to complete and it sits with your other nursing credentials.

Why does a facility need one if you are a contractor?

Because you are in their building. Employment status does not change what happens if a clinician is injured on a shift, becomes unwell, or is caught up in an incident on the premises. The facility needs a name and a number without having to go looking for one. It is one of the few credentials on your list that exists for your benefit rather than to establish that you are qualified.

Who should you list?

Someone who will actually answer. That sounds obvious until you work nights and remember that the person you listed silences their phone at 10.

  • Whether they are reachable during the hours you usually work
  • Whether they could reach you or make arrangements if it came to that
  • Whether a second contact is worth adding as a backup

Tell the person you have listed them. Being called by a facility they have never heard you mention is a poor way to find out.

What happens if it goes out of date?

It quietly stops working. Numbers change, relationships change, and an emergency contact form is the kind of document nobody revisits until the moment it matters. Check yours whenever you update anything else on your profile, and after any change that would make the listed person the wrong call.

Adding it to your profile

Complete the form and upload it.

  1. Tap Menu in the top left of the Nursa app.
  2. Select Profile.
  3. Scroll to the Credentials section.
  4. Tap Add your Credential.
  5. Upload your document as a PDF, WORD, JPG, or PNG.
