HCA | Medical History Questionnaire

A medical history questionnaire is part of the health screening HCA runs before a clinician works a shift. It is a form you complete rather than training, and it sits with your other nursing credentials once done.

What does the questionnaire ask about?

Health information relevant to the work. Expect questions covering immunization history, past tuberculosis testing or exposure, conditions affecting your ability to perform physical tasks, and any respiratory conditions. The scope follows the demands of clinical work rather than amounting to a general medical history.

Who actually sees your answers?

Occupational health, not the unit. Health screening information is handled separately from the rest of your file, and the people deciding whether you can work a shift generally receive a determination rather than the answers behind it. That separation is deliberate. It exists so a clinician can answer accurately without medical details circulating among managers and schedulers.

How does this connect to respirator use?

Directly, in many cases. Anyone required to wear a fitted respirator such as an N95 needs a medical evaluation first, because breathing through one adds work and some conditions make that unsafe. The evaluation is usually a questionnaire reviewed by a licensed health professional, and what gets reported back is whether you are cleared to wear one rather than what you disclosed. Fit testing comes after clearance rather than instead of it.

What if something changes later?

Tell occupational health rather than waiting for the next screening. A new diagnosis, a surgery, or a change in medication can affect a fitness determination or a respirator clearance, and a questionnaire you completed a year ago describes who you were then. Updating it is quicker than starting over.

  3. Scroll to the Credentials section.
  4. Tap Add your Credential.
  5. Upload your document as a PDF, WORD, JPG, or PNG.
