IGRA | Tspot or Q-gold within last 12 months

IGRA is a blood test for tuberculosis infection, sold as T-SPOT and QuantiFERON-TB Gold. Facilities commonly ask for a result from within the past 12 months, and it sits with your other nursing credentials.

How is it different from a skin test?

One visit instead of two. A skin test means an injection and a return 48 to 72 hours later for someone to read it, and the reading involves judgment. An IGRA is a single blood draw with a laboratory result, so there is nothing to come back for.

Why does BCG vaccination matter here?

Because it distorts the other test. Clinicians vaccinated with BCG, which is routine across much of the world, frequently return a positive skin test reflecting the vaccine rather than infection. IGRA is not affected by BCG, which is why it is usually the test worth asking for if you were raised or trained outside the United States.

What happens if the result is positive?

Further evaluation rather than a conclusion. A positive IGRA indicates TB infection, not active disease, and the next step is assessment including symptom review and imaging to establish which it is. It also changes your testing from then on, since someone with a documented positive is generally not retested and moves to symptom screening instead. Work through a positive result with a provider or occupational health rather than reading conclusions into the report.

Why within the last 12 months?

Because that is facility policy rather than a universal rule. Current national guidance for healthcare personnel centers on baseline testing and testing after a known exposure rather than routine annual testing. Plenty of facilities still ask for an annual result, so what a shift requires is what governs.
