Intermountain | MedSurg Modules and Skills Assessment Form

This credential pairs medical-surgical modules with a skills assessment form. The modules cover knowledge, the form records what you are competent to do, and both are wanted before you take a med-surg assignment. Together they sit with your other nursing credentials.

What do the modules cover?

The breadth a medical-surgical floor carries.

  • Assessment and reassessment across common medical and post-surgical presentations
  • Wound care, drains, and surgical site monitoring
  • Medication administration at volume, including IV therapy
  • Recognizing deterioration without continuous monitoring
  • Fall prevention, mobility, and skin integrity
  • Discharge teaching and transitions of care

What makes med-surg different from a specialty unit?

Numbers and breadth rather than acuity. A specialty unit gives you 1 or 2 patients and a narrow set of problems. A medical-surgical floor gives you 5 or 6 whose diagnoses may have nothing in common, and none of them on a continuous monitor. Catching a change means noticing it yourself, on a round, between everything else that is happening. The core skill is not any single procedure. It is deciding what to do next, repeatedly, for the length of a shift.

What should you ask before taking the assignment?

Things that shape a shift more than the diagnosis list does.

  • How many patients you will carry and what the usual ratio is
  • Whether aides are on the floor, and how many
  • Who the charge nurse is and whether they carry patients too
  • How many admissions or discharges are expected
  • Where to find the resource nurse if the floor gets away from you

None of those are difficult questions, and all of them are easier asked at the start of a shift than at hour 6.

Adding it to your profile

Upload both parts, the module completion and the signed skills assessment.

  1. Tap Menu in the top left of the Nursa app.
  2. Select Profile.
  3. Scroll to the Credentials section.
  4. Tap Add your Credential.
  5. Upload your document as a PDF, WORD, JPG, or PNG.

Sign up as a clinician and keep your med-surg and specialty forms separate on your profile, since a facility checks for the one matching the unit.
