This credential covers three vaccination records in one: MMR (measles, mumps, rubella), Tdap (tetanus, diphtheria, acellular pertussis), and hepatitis B. Facilities that require one commonly require all three, and they sit together with your other nursing credentials.
Why are the three grouped together?
Because they are the most common adult vaccine requirements for health care workers. Most facilities require annual influenza vaccination on top of these, but that one operates on its own schedule, while MMR, Tdap, and hepatitis B are a static requirement. The specific criteria differ between them, but a facility that requires one will usually want all three.
What does each vaccination require?
The details vary, so check your documentation against the list.
- MMR: two doses, or a positive titer
- Tdap: one dose as an adult, regardless of last Td booster
- Hepatitis B: a three-dose series, or a positive titer, or a signed declination
If your facility requires two specific MMR doses, a positive titer on its own may not be enough. And some facilities do not accept a hepatitis B declination. Read what your credential list asks for rather than assuming.
What if your childhood records are missing?
There are a few places to try before starting over. Your state immunization registry may hold your records, even from years ago. Your parents may have a copy from when you were a minor. Your high school or undergraduate records may reflect your status at enrollment. Past employers, particularly health care employers, sometimes hold an archive. It is worth checking a few places before you conclude the records are gone.
Adding it to your profile
Upload your vaccine records or titer results, making sure your name and the date of each dose or draw appears clearly. If you are declining hepatitis B vaccination, upload the signed declination form.

- Select Profile.
- Scroll to the Credentials section.
- Tap Add your Credential.
- Upload your document.
