A certified medication technician working with Nursa administers medications and supports resident care under the supervision of a licensed nurse. As a PRN CMT, your responsibilities may vary from one work setting to another. PRN medication technicians work in a variety of healthcare settings, such as:

  • Skilled nursing facilities (SNFs)
  • Nursing homes
  • Assisted living (AL)
  • Residential care homes
  • Memory care units
  • Adult day care centers
  • Community-based and home settings (in some states, under nurse delegation)

CMT responsibilities

CMTs work in different environments, and while their duties vary from one setting and state to another, the following are general responsibilities. As a per diem CMT, you must be adaptable and ready to integrate seamlessly into various facility teams:

Medication administration

CMTs administer medications and support safe medication practices, including:

  • Set up and administer non-parenteral medications—such as oral, topical, ophthalmic, otic, nasal, and inhaled medications—to the correct patient at the correct time.
  • Follow the rights of medication administration on every pass, verifying the right patient, medication, dose, route, time, and documentation.
  • Prepare medications from properly labeled containers and verify each against the medication administration record (MAR).
  • Administer medications only within the scope permitted by state regulation and delegated by the supervising nurse.

Observation, monitoring, and safety

CMTs monitor patients for medication effects and help maintain a safe environment:

  • Take and record basic measurements—such as vital signs—before administering related medications (where trained and delegated).
  • Observe patients for expected effects, side effects, and adverse reactions following medication administration.
  • Report physical or behavioral changes, refusals, or concerns to the supervising nurse promptly.
  • Adhere to universal precautions, infection control standards, and correct use of personal protective equipment (PPE).
  • Recognize medication-related emergencies and summon the supervising nurse or emergency help per facility protocol.

Medication management

CMTs help keep medications accurate, secure, and available:

  • Store and secure medications, including controlled substances, properly per facility and state protocols.
  • Perform medication cart audits and help maintain accurate counts.
  • Coordinate with the supervising nurse and pharmacy on medication availability, reorders, and discrepancies.

Documentation and confidentiality

CMTs document care accurately while protecting patient privacy:

  • Document each administration, omission, or refusal objectively and accurately in the MAR or EHR/EMR.
  • Report medication errors, incidents, or unusual occurrences promptly and in accordance with facility procedure.
  • Communicate relevant medication information at shift change to support continuity of care.
  • Maintain patient confidentiality and safeguard all protected health information (PHI) in accordance with HIPAA.

Physical and emotional demands

CMTs should be able to meet the following physical and emotional demands:

  • Stand and walk for extended periods during medication passes.
  • Perform safe patient handling and assist with moving or repositioning as required by the facility.
  • Provide compassionate and reliable care.
  • Communicate effectively with the healthcare team.
  • Cope with high-stress situations.

CMT qualifications

The following qualifications, compliance requirements, and skill standards are typically expected of CMTs who pick up shifts with Nursa:

  • Certification: Hold an active, unencumbered state medication technician certification (CMT), valid and listed on your state's registry.
  • Education: Hold a high school diploma or GED.
  • Certifications: Maintain Cardiopulmonary Resuscitation (CPR) and/or Basic Life Support (BLS) certification where required by the facility.
  • Experience: While requirements vary, recent experience administering medication is often preferred.
  • Background check: Pass required local, state, and federal background checks, including caregiver abuse registry screenings.
  • Vaccines: Provide proof of up-to-date immunizations.
  • Fit-to-work: Some facilities may require a Fit-to-Work statement, TB test, or physical exam.

CMT skills

  • Use electronic health/medical record (EHR/EMR) and medication administration record (MAR) systems for accurate and timely documentation.
