Some facilities accept either proof of influenza vaccination or a signed refusal form. Both satisfy the credential, and either sits with your other nursing credentials once filed.
What does a refusal form do?
It documents a decision rather than leaving a gap. A declination form records that the vaccine was offered and that you chose not to receive it, usually with an acknowledgment that you have read the facility's information. Filing one satisfies the credential wherever the requirement is written as vaccination or refusal. Doing nothing does not, and that is the distinction worth understanding, because an unanswered requirement reads as incomplete rather than as a choice.
What usually comes with declining?
Conditions, often. Facilities commonly ask staff who decline to wear a mask through the respiratory season, and some limit assignments during an outbreak. Those are facility policies rather than anything contained in the form, so the practical effect varies from building to building. Ask what applies before the season rather than finding out at the start of a shift.
Does a refusal filed at one facility carry to another?
Generally not. Declinations are handled by the facility that offered the vaccine, so a form signed at one building says nothing about the requirement at the next. Clinicians working several facilities may file more than one. Some facilities do not offer a refusal option at all, in which case vaccination is the only route to that credential.
Does either one carry over to next season?
No. Both the vaccination record and the refusal form are tied to a single influenza season, since the vaccine is reformulated each year and the offer is made again. Whatever you filed last autumn does not satisfy this autumn, so expect to revisit it annually.
