Intermountain | Department Orientation Checklist

A department orientation checklist is the unit-level layer of orientation. Unlike modules you work through on your own, this one gets completed on the department itself and signed off as you go. It joins your other nursing credentials once done.

How is this different from facility orientation?

Scale. Facility orientation covers the building, its codes, its policies, its evacuation routes. A department checklist covers one unit inside it, down to where the crash cart sits, how report is given, and who takes the call when a patient deteriorates at 3 in the morning. Building orientation tells you the rules. The department checklist tells you the room.

Who signs it, and when?

Someone working the unit, usually at the start of your first shift there or during a scheduled orientation shift. That is what sets this checklist apart from everything else on your credential list. You cannot complete it at home the night before. It needs a person on the department with time to walk it with you, which means arriving early and knowing in advance who is expecting you.

What if nobody has time to go through it with you?

Work it yourself, then get it signed. Units are busy and the person meant to orient you may be carrying a full assignment. Rather than abandoning the checklist, walk the unit with it in hand, find the items you can find, note the ones you cannot, and ask the charge nurse to confirm and sign. An honest checklist with 2 unanswered lines is worth more than a complete one nobody actually checked. If something on it matters to safe practice and stays unanswered, say so before you take an assignment rather than after.

Adding it to your profile

Upload the completed checklist with the signatures visible.

  1. Tap Menu in the top left of the Nursa app.
  2. Select Profile.
  3. Scroll to the Credentials section.
  4. Tap Add your Credential.
  5. Upload your document as a PDF, WORD, JPG, or PNG.
