The Long Term Care (LTC) Skills Self Assessment Form is a clinical self-evaluation tool used to document a clinician's experience and proficiency across long-term care procedures. Facilities rely on this assessment to match clinician skill levels with resident acuity, ensuring safe, compliant, and continuous high-quality care across skilled nursing, assisted living, and memory care settings.
What does the assessment cover?
The self-assessment evaluates clinical comfort levels and hands-on experience across primary long-term care domains:
Basic and advanced nursing procedures
- Clinical interventions: Experience with tracheostomy care, enteral feeding tubes (G-tube/J-tube), wound care dressings, catheter insertion, and ostomy management.
- Vital signs and monitoring: Frequency of conducting routine physical assessments, oxygen therapy administration, and blood glucose monitoring.
Medication administration and charting
- Medpass routines: Proficiency with oral, topical, subcutaneous, and intramuscular medication passes.
- eMAR & Controlled Substances: Navigating digital charting platforms, executing shift narcotic counts, and logging controlled drug waste.
Activities of daily living and mobility support
- Patient transfers: Operating mechanical lifts (Hoyer lifts), standing assistance devices, and gait belts safely.
- Personal care: Assisting residents with daily hygiene, feeding, range-of-motion exercises, and fall prevention protocols.
Why is the self-assessment required?
Long-term care residents often present complex, chronic health conditions that require specialized daily management. Completing an accurate self-assessment provides facility leaders with clear visibility into your clinical experience. This supports balanced shift assignments, prevents safety gaps on the unit, and ensures you are placed in roles where you can practice confidently.
What does it look like on a shift?
Having an accurate skills self-assessment on file allows unit managers to assign residents whose clinical needs align with your verified expertise. On shift, it ensures smooth collaboration with nursing staff and minimizes confusion during shift handoffs or medication passes.
