Certified Nursing Assistant CNA - Skilled Nursing $24.09 per hour

ANY CLINICIAN REQUESTING SHIFTS AT ESCONDIDO POST ACUTE IS ACKNOWLEDGING AND AGREEING TO THE FOLLOWING:

I, CLINICIAN NAME, am recognizing and agreeing to complete ALL duties of a CNA. Responsibilities related to patient care INCLUDE documenting care, or otherwise known as “charting”, in PCC. 

As a 1099 clinician it is still MY responsibility to complete charting that is done for patients while under my supervision. Facilities are required to provide MY documentation when audited by state and federal compliance organizations. IT IS MY DUTY TO CHART and I will not end my shift or leave the facility until ALL CHARTING IS COMPLETE.

Failure to comply with proper documentation or “charting” can result in the following:

1. Delayed Payment
2. Requirement to come back to facility to complete charting
3. Nursa Suspension (inability to complete future shifts at any facility in the Nursa Application)
4. Blocked from Facility 

Please follow these instructions: Requesting clinicians should have TB SCREENING, PHYSICAL, CPR CARD 

1. As soon as you arrived in the facility, please check in to Station 1 and sign the staffing binder. In the binder you can see what stations you're going to and write down your patient's room numbers under the "assignment" on the 530 form.

2. 530 forms are located in the registry binder at station 1.

3. Please make sure you chart (ask Zandy for charting login info) before you leave the facility and acknowledge the Charting Acknowledgement that is attached to this shift posting.

4. At the end of the shift fill out your 530 form and have the RN Supervisor sign it. 5. 530 forms are found in registry binder next to assignment binder and will not be signed if charting is not completed. Please place completed 530 in registry binder under correct date tab.
